**Country/Authority that issued passport:** This should be the country you are a citizen of, even if you received your passport while in another country.

**Where was the passport issued, Issuance date, Expiration date:** These details should be accurate and match your passport. Note that your passport must have at least six months of validity beyond your visa application and/or arrival in the U.S.

**Have you ever lost a passport or had one stolen:** If yes, provide the necessary details. Misrepresentation or omission of this information can lead to serious consequences.

Remember, the visa will be issued in the name as it appears in your passport. If you want a different name (e.g., your married name), you must first change your passport. If your entire name is written in the field for surname, and the given name field is left blank, the consulate will treat your entire name as your surname and write “FNU” (First Name Unknown) in the first name section. If you do not want “FNU” or “LNU” (Last Name Unknown) as part of your name on your visa, you should get your passport changed before applying for the visa.

**Principal Applicant’s Company Name:** This is only required if the applicant is not the principal applicant and the trip purpose is Treaty Trader/Investor (E1, E2).

**Intended Date of Arrival, Intended Length of Stay in the U.S.:** If uncertain about your travel plans, provide an estimated date and duration of stay.

**Address where you will stay in the U.S.:** This could be a hotel address if you don't know where you'll be staying upon arrival. If you're employed in the U.S. and maintain an apartment there, provide that address and telephone number.

**Person/Entity Paying for Your Trip:** If you're paying for your trip or a U.S. petitioner is doing so, no further questions will be asked. However, if someone else is paying for your trip, you'll need to provide their name.

This section is about the people traveling with you, if any. 

**Are there any other persons traveling with you?** If you're traveling with family members, you'll need to provide their full names and their relationships to you.

**Are you traveling as part of a group or organization?** If you answer "yes," provide the group or organization's name. If you answer "no," provide the surnames/last names, given names, and relationships of the persons traveling with you. If more than one person is traveling with you, click "add another." You can prepare a family/group application once you're done with the first applicant's application. All of you will be interviewed at the same time.

**Are you applying in the same country where the visa above was issued and is this country your principal country of residence?** Again, answer "yes" or "no."

**Have you been ten-printed?** Ten-printed means that you have provided fingerprints of all your fingers, as opposed to only two fingerprints.

**Has your U.S. visa ever been lost or stolen?** If "yes," provide the year it was lost or stolen.

**Has your U.S. visa ever been cancelled or revoked?** If "yes," provide an explanation.

**Have you ever been refused a U.S. visa, been refused admission to the U.S., or withdrawn your application for admission at the point of entry?** If "yes," provide an explanation. 

**Has anyone ever filed an immigrant petition on your behalf with the United States Citizenship and Immigration Services?** If "yes," provide the details in the explanation box.

Your U.S. point of contact can be any individual in the U.S. who knows you and can vouch for you. If you don't know anyone in the U.S., you can enter the name of the company or organization you plan to visit during your trip.

**Contact Person, Organization Name, Relationship to You, Address and Phone # of Point of Contact, Email Address:** These fields are self-explanatory. Remember to provide accurate and truthful information. Any discrepancies could result in visa denial or other issues.

This section of the DS-160 form is crucial for assessing your eligibility for a U.S. visa. If you answer "yes" to any of these questions, you may be denied the visa. It is advisable to seek advice from an immigration attorney if you have questions regarding this section.

If you indicated that the purpose of your trip to the U.S. is to work temporarily, provide additional information about your employer.

If the applicant is under 16 or is unable to electronically sign the form for any reason, their parents, guardians, or other individuals who have legal custody over them must submit the form on their behalf.

Once you complete the online form, print the confirmation sheet with a barcode. This will allow the consular officer to access the information contained in the DS-160. You must print the [confirmation page](https://ceac.state.gov/genniv/^) with a laser printer so the barcode can be properly read.

If the confirmation page includes the image of your photograph, the photo was uploaded successfully. If you see an "x" in the box where the photo should be, it means that the photo upload has failed, and you will need to submit one printed photograph along with the confirmation page of the online DS-160.

You do not need to take the printout of the DS-160 form itself. You can save the DS-160 form data for your own records.
